Please Read The Following Instructions:
- Click the link above to access the Neuropsych Questionnaire (NPQ).
- If prompted, install the Microsoft Silverlight plugin.
- Once the application loads, click "Tests" along the top.
- You will be asked to enter information for the patient that will be seen in the clinic.
- We encourage other family members or teachers to also complete this assessment of the patient.
- Once you have completed the assessment, your results will be sent to our office.
